This document serves as a liability release, pursuant to which {name} agrees to
indemnify and hold Grant's Martial Arts Academy, Limited (hereby referred to as "ACADEMY") harmless against any and all claims of loss or damage
without limitation.
{name} expressly acknowledges that he/she will be engaging in martial arts, physical
exercises, and activities while attending the ACADEMY facilities, which could cause injury to
{name}.
{name} hereby states he/she is and will be voluntarily participating in
these martial arts, physical exercises, and activities and {name} hereby assumes all risks
of injury which might result from these physical exercises and activities.
{name} hereby waives and releases any and all claims that he/she now has or may have
against the ACADEMY, its employees or agents for injury sustained by {name} as a result
of these martial arts, physical exercises, and activities. In the event of an injury, {name}
must immediately notify the management of the ACADEMY facility, which they attend, and
provide full written particulars of the injury and the accident upon being requested to do so.
{name} hereby releases and holds harmless the ACADEMY, its agents, officers, and employees, and any affiliated companies from any liability with respect to injury to {name} or {name}’s property arising out of, or connected with the {name}’s physical exercise and activities.
{name} hereby waives and releases the ACADEMY from any claims he/she may have
against the ACADEMY with respect to any loss or theft of personal property in respect of the
facilities operated by the ACADEMY.
{name} hereby warrants and represents that he/she is in good physical condition and
that he/she has no liability, impairment preventing him/her from engaging in active or passive
exercise that will be detrimental or injurious to his/her health, safety, comfort and physical
condition or that of other(s).
{name} hereby represents that he/she will not use any ACADEMY facilities or services
while having any open cuts, abrasions, weeping sores, infections, illnesses, or any other
physical impairments, which could be aggravated by the use of the facilities. The management
of the ACADEMY shall have the right to make a final binding determination in that regard.
{name} hereby acknowledges that participation in activities at the ACADEMY may involve the recording and live-streaming of images, video, and audio (the "Materials").
By signing this waiver, {name} grants the ACADEMY the right to record and use their image, likeness, voice, and statements in the Materials. This includes the right to use, reproduce, edit, publish, broadcast, and distribute the Materials in various media for any lawful purpose, including promotion. This authorization is irrevocable and applies to all languages, media, formats, and markets, continuing indefinitely unless revoked in writing.
{name} acknowledges that the ACADEMY will own all rights to the Materials and waives any right to inspect or approve the final product. {name} also waives any right to receive compensation for the use of the Materials.
Participation is voluntary. {name} releases the ACADEMY from any liability, claims, or damages that may arise from the authorization or use of the Materials, including claims related to defamation, privacy, or copyright.
{name} hereby acknowledges that he/she has carefully read this waiver and release and
fully understands that it is a release of liability of the ACADEMY and agrees that such a waiver
and release is responsible and proper based on the nature of the ACADEMY’s business.
If any provision of the agreement is determined to be invalid or unenforceable in whole or in part, such invalidity or unenforceability shall pertain only to such provisions and everything else in the agreement shall continue in full force and effect.
